Fees & Health Insurance
- Initial session $200 / additional sessions $170
- Payable by cash, check or PayPal at the time of session.
- I am out of network with all insurance companies.
- Contact your health insurance to request information about "out of
network benefits for outpatient mental health office visit".
- Tell them codes for billing: CPT 90791 (60 min initial intake & consult)
& CPT 90834 (45-50 minute additional psychotherapy sessions)
- At the end of each month, I will forward a statement via email for
submitting to health insurance for reimbursement if eligible.
Scheduling
Office hours are on Tues & Thurs 9-4pm. No evenings or weekends.
Sessions are conducted in person and via Zoom.
